Parent/Guardian Verification of Eligibility for
Free and Reduced Price School Meals
In order for your student to qualify to be enrolled in Utah’s Statewide GEAR UP Education Program, they need to be eligible under the Free and Reduced Price School Meals Guidelines. Please review the chart below and refer to it as you complete the rest of the form.

If your student is eligible, you may be asked to provide income verification that you meet income eligibility requirements. We randomly select families to verify their income. If your family is selected, we will notify you about the income documentation you will need to provide for verification.
*Please note that this form is not an Application for Free & Reduced Price School Meals, but rather an eligibility verification form for the GEAR UP Program. If you would like to apply for the Free & Reduced Price School Meals Program, please contact your student’s school.
